This MP3 audio recording is a live Ayurvedic Chinese Medicine health consultation with an actual patient who has a variety of symptoms including chronic sinusitis, sinus congestion, kapha spring allergies, frequent colds, migraines, fatigue, hypersomnia, hemorrhoids and some heart burn (acid reflux, gastro-esophageal reflux - GERD).

The consultation provides you the compassionate listener and the patient with an introductory level yet detailed analysis of their health focusing on diagnosis of Prakruti (genetic constitution of vata-pitta-kapha) and Vikruti (imbalance of three doshas of vata-space-air-movement, pitta-fire-water-metabolism, kapha-water-earth-phlegm-fat).

The patient’s anonymity is preserved with no reference to their name, phone number, e-mail or any other specific identifying information (see below at the end of this text for the agreement between the patient and the monk practitioner).

The practitioner examines the patient from the perspectives of both Traditional Chinese Medicine (T.C.M. or Traditional Oriental Medicine and Acupuncture) and Indo-Tibetan Buddhist and Hindu Ayurveda and Tibetan Medicine (Traditional Ayurvedic Medicine - T.A.M.).

The practitioner interviewing the patient is from the Medicine Buddha Healing Center -- an anonymous American Buddhist Monk of the Nagarjuna Nalanda Tradition of Indian - Chinese - Tibetan Buddhism - Buddhist Ayurvedic Studies Program Distance Learning -- http://www.Ayurveda-California.com. This American Monk studied with renowned Ayurvedic physician Dr. Vasant Lad, B.A.M.S., M.A.Sc. for a six-year 1800-hour 1900-patient Ayurveda apprenticeship. This American Monk has served over 5500 patients over the last 11 years. This Monk Practitioner has also studied Chinese Medicine for 2 years in both Albuquerque (IICM) and the Bay Area (ACCHS and with private tutors).

These high quality MP3 audio files of Ayur-Vedic Chinese Healing Arts Health Consultations are the first in a 3 trimester unit (45 class hours long) series of lectures aimed and comparing, contrasting and synthesizing the 2500 year old practice of Chinese Medicine (Zhong Yi or TCM) with the 2500 year old Indian Ayurveda and 1300 year old Tibetan Ayurveda systems (TAM). The goal of this lecture program series is to assist the student in seeing and understanding their vast commonalities in both theory (concepts) and practice (clinic) and to understand by using real word patients how the Asian healing arts can help people at a practical level.

This rarely presented clinical synthesis of the healing wisdom of these three great cultures (China, India and Tibet) will not only compare-contrast the theory of these two time-honored healing systems, but more importantly, will examine in detail the clinical practice aspects -- both herbal connections (Indian, Tibetan and Chinese herbs and minerals) and acupressure (Marmas in Ayurvedic Sanskrit) with acupuncture in the Chinese system.

By using actual patient visits (clinical consultations), we examine the three doshas (Tridosha - Vata-Pitta-Kapha) relationship to major concepts in Chinese Medicine that are often spoken of by acupuncturists to their patients. Some of these relationship comparisons include:
1. Vata Dosha (Space and Air - Wind, Cold, Qi [Prana in Ayurvedic Sanskrit]),

2. Pitta Dosha (Fire and Water - Spleen Qi [Jathar Agni in Sanskrit], Heat, Damp-Heat, Blood [Rakta Dhatu in Sanskrit], and Yang),

3. Kapha Dosha (Water and Earth - Phlegm-Mucous [Ama in Sanskrit], Damp-Cold, Jing Qi [Ojas in Sanskrit] and Yin).

Commentary on and Lectures in a personal way with the actual patient comes from the ancients texts of Indo-Sino-Tibetan Medicine: Chinese Classic of the Yellow Emperor - Huang Di Nei Jing (~200 B.C.), Shang Han Lun (~150 A.D. traditional Chinese: 傷寒論; pinyin: Shāng Hán Lùn), Divine Farmers Materia Medica (~150 A.D. The Divine Farmer's Herb-Root Classic (traditional Chinese: 神農本草經; pinyin: Shénnóng běncǎo jīng) with Charaka of Patanjali (~200 B.C.), Sushruta of Nagarjuna (~200 A.D.), Astanga Hridayam of Vagbhata (~700 A.D.) and the "Four Tibetan Medical Tantras" (~800 A.D. "rGyud - bzhi" in Tibetan, pronounced "Ju Shee" --- "Si-Bu-Yi-Dian in Mandarin --- in Sanskrit it is called the "Amrta-hrdaya-astanga-guhyopadesa-tantra" or Amrita Hridaya Astanga Guhyopadesha Tantra)

It would useful, but not necessary, that you also listen to the 7 half hour preliminary HDTV video lectures (usually 30 minute long for each video) found at:

Also, recommended is the Chinese Medicine comparison with Ayurveda at:
http://www.archive.org/details/TCM401_Ayurvedic_Chinese_Medicine_001_Comparison
http://www.archive.org/details/TCM401_Ayurvedic_Chinese_Medicine_002_Comparison http://www.archive.org/details/TCM401_Ayurvedic_Chinese_Medicine_003_Comparison

If not, you will miss out on the broader big picture foundation explanations of 1. Vata, 2. Pitta and 3. Kapha Doshas and their subtypes and their comparison-contrast with the Chinese Medicine concepts of 1. Qi - Wind - Cold - Yin Deficiency; 2. Yang - Spleen-Qi - Heat - Damp-Heat - Blood - Excess; and 3. Yin - Fluids - Phlegm-Mucous - Damp-Cold - Jing Qi. This first three videos are required preparations lectures for the more advanced and clinical oriented lectures that compare the two countries herbal and acupressure-puncture systems.
The more advanced lectures after these initial 3 videos classes (listed above) provide a detailed introduction that builds on these basics by focusing on diagnosis of Prakruti (genetic constitution of vata-pitta-kapha compared with modern Chinese Five Element typing) and Vikruti (imbalance of three doshas of:

1. VATA -- Space-Air Elements -- Movement and Catabolism -- Organs like colon - large-intestine - triple-warmer-circulation - Mind (Prana Vayu in Sanskrit),
2. PITTA -- Fire-Water Elements -- Digestion and Metabolism -- Organs like liver-gallbladder - small-intestine - Mind (Sadhaka Pitta), and
3. KAPHA -- Water-Earth Elements -- Growth and Anabolism -- Phlegm-Fat-Minerals - Organs like lungs-stomach-heart-kidneys-urinary-bladder - Mind (Tarpaka Kapha).

compared with imbalances of:

1. QI -- Wind - Cold - Yin Deficiency - TCM Element Metal;
2. YANG -- Spleen-Qi - Heat - Damp-Heat - Blood - Excess - TCM Elements Fire and Wood; and
3. YIN -- Fluids - Phlegm-Mucous - Damp-Cold - Jing Qi - TCM Elements Water and Earth).

Although we help the patient by using the clinical wisdom from the ancient Chinese medical classics, we rely heavily on the two modern TCM textbooks that are popular in the colleges of Chinese medicine in the USA and Europe:
1. Maciocia, Giovanni, Foundations of Chinese Medicine: A Comprehensive Text for Acupuncturists and Herbalists, 2nd edition, Published by Churchill Livingstone, First published: 1989. Medicine Master Buddha Mantra (Yao Shi Fou):
“Om Namo Bhagavate Bhaisajya Guru Vaidurya Prabaha Rajaya Tathagataya Arhaté Samyaksam Bodhi Tadyata Om Bhaisajé Bhaisajé Bhaisajya Samudgaté Svaha.”

“Om Namo Bhagavate Bhaisajya Guru Vaidurya Prabaha Rajaya Tathagataya Arhaté Samyaksam Bodhi Tadyata Om Bhaisajé Bhaisajé Bhaisajya Samudgaté Svaha.”
Namo Four-Unlimited-Minds - Four Immeasurables - Four-Far-Reaching-Attitudes - Namo Chatur- Maha-Brahma-Viharas- great loving-kindness (Namo Maha Maitri), great compassion (Namo Maha Karuna), great sympathetic joy (Namo Maha Mudita), and great renunciation-detachment-equanimity (Namo Maha Upeksha).

The Four immeasurable Thoughts
Namo Maha Upeksha: How wonderful it would be if all sentient beings were to abide in equanimity (Maha Upeksha), free from hatred and attachment, not holding some close and others distant.
May they abide in equanimity. I myself will cause them to abide in equanimity.
Please, guru-deity, bless me to be able to do this.

Meditate briefly on tong-len (exchanging self for others), taking on all the disturbing emotions (tri-kleshas - three poisons of vata windy anxiety raga klesha, pitta fiery anger dvesha klesha, kapha sticky muddy-muddled ignorance moha klesha) of sentient beings and giving them to your self-cherishing thought, thus destroying it.

Namo Maha Mudita: How wonderful it would be if all sentient beings had happiness and the causes of happiness.
May they have happiness and its causes. I myself will cause them to have happiness and its causes.
Please, guru-deity, bless me to be able to do this.

Meditate briefly on tong-len (exchanging self for others), transforming your body into a wish-granting jewel (chittamani) that gives every sentient being every kind of temporal and ultimate happiness that they desire and need.

Namo Maha Karuna: How wonderful it would be if all sentient beings were free from suffering and the causes of suffering.
May they be free from suffering and its causes. I myself will cause them to be free from suffering and its causes. Please, guru-deity, bless me to be able to do this.
Meditate briefly on tong-len (exchanging self for others), taking on the sufferings and causes of suffering of all sentient beings.

Namo Maha Maitri: How wonderful it would be if all sentient beings were never separated from the happiness of higher rebirth and liberation. May they never be separated from these. I myself will cause them never to be separated from these. Please, guru-deity, bless me to be able to do this.

Meditate briefly on tong-len (exchanging self for others), giving all one's merits of past, present, and future and their results, including enlightenment, to all sentient beings.

LC 4 For the sake of all mother sentient beings, I shall become a guru-deity (Bodhisattva or Buddha)
And place all sentient beings In the supreme state of a guru-deity. (3x)
Meditate here on generating special bodhichitta in order to enter into the practice of Mantra - Highest Yoga Tantra (Maha Anuttara Tantra).

Generating Special 'Bodhichitta.
LC 5 For the sake of all mother sentient beings I shall quickly, quickly, in this very life, Realize the state of a primordial guru-yidam Buddha
(1. Namo Tathagata Kulaya: Namo Vairochana [Vairocana 吠路者那, 毘盧舍那] - Nishyanda-Shakyamuni [Śākya-muni 釋迦如來, 釋迦牟尼, Śākyamuni Tathāgataḥ 釋迦如來];
2. Namo Padma Kulaya: Namo Vajradharma-Amitayus-Amitabha [Amitābha 無量光, 阿彌, 阿彌陀, 阿彌陀婆, Amitāyus 無量光佛, 無量壽佛, 阿彌陀, 阿彌陀廋斯];

LC 6 I shall free all mother sentient beings from suffering And place them in the great bliss of Buddhahood; Therefore, I shall now practice The profound path of guru-yidam yoga.

Dissolve and absorb the refuge merit field and meditate on the three kayas (Om-Ah-Hum -- 1. OM Dharma-Kaya - Dharma-Dhatu - Tathagata-Garbha - Buddha-Matrix, 2. AH Sambhoga-Kaya - Reward Body - Enjoyment Body, 3. HUM Nirmana-Kaya - Transformation Body)

"The Dharani true words Nan Ya Hung (Om Ah Hum) are Vairochana, Amitabha and Akshobhya Medicine Buddha. The Dharma, Reward and Transformation bodies' three great meanings: All sages and worthies of the ten directions come forth from these." (From Ven. Master Hsuan Hua's Shurangama Commentary) OM AH HUM HA HO HRI

Because I, the deities, the field of merit, and all other phenomena are imputations (merely labeled - shunyata) on dependent arisings (pratityasamutpada), everything becomes empty having the true nature of selflessness (anatman), free from the four extremes such as eternalism, nihilism, and so forth.

OM SVABHAVA SHUDDHA SARVA DHARMA SVABHAVA SHUDDHO HAM
OM SHUNYATA JNANA VAJRA SVABHAVA ATMAKO HAM

Pause to meditate here on emptiness.
Then arise in the form of Medicine Buddha (Bhaisajya Guru)
or as whatever highest yoga tantra deity you have been initiated into.

LC 1 From within great bliss (Maha Sukha Samvara), I arise as a guru-deity.
From my clear body, masses of light rays radiate into the ten directions,
Blessing the world and its beings.
All becomes perfectly arrayed with only infinitely pure qualities.

Pastoral Counselor’s and Client’s Declarations of First, Fourth, Sixth and Ninth Amendment Constitutional Rights and of Pastoral Counseling Authorization Contract / Agreement and Informed Consent

We declare that the ancient Buddhist Ayurvedic Healing Arts (Buddhist Healing Arts) of India-Tibet offer health education (pastoral counseling) to modern civilization to keep the proper balance between body, mind and consciousness (spirit). Through proper lifestyle, diet, cleansing regimes, and rejuvenation programs the Buddhist Healing Arts have traditionally helped one establish a long, healthy, fruitful and spiritual life. These healing arts were first described in the Sanskrit language in the ancient Buddhist, Yogic and Vedic scriptures and sutras and their commentaries such as the Medicine Master Buddha Sutra, the Dharani Sutra, the Shurangama Sutra, the Ambrosia Heart Tantra, and the Contemplation of King of Healing and Supreme Healer Bodhisattvas Sutra, Charak, Sushrut, and Vagbhat and others. We declare that the Buddhist Ayurvedic Healing Arts, although recently introduced to the United States, have been practiced by both monastic and lay practitioners in India for 2500+ years (since the time of Shakyamuni Buddha), and in Tibet for 1400+ years (since the arrival of Buddhism). It is, however, not presently recognized by the medical establishment of this country and such Buddhist Healing Arts are alternative or complementary to healing services licensed by the state. Buddhist Healing Arts are not and may not be licensed by the state.

I, _________________________________, acknowledge that the monk Venerable ________________ is a Buddhist Pastoral Counselor and Ayurvedic Herbalist, commissioned by the New Mexico-based and California-based non-profit 501(c)3 church, called the Medicine Buddha Healing Center (also know as the Ayurveda Healing Arts Institute) and he is not a licensed physician. I acknowledge that he studied for 6 years, and more than 1800 formal classroom and clinic hours, seeing 1900 clients with renowned Ayurvedic Physician Dr. Vasant Lad, M.A.Sc., at the Ayurvedic Institute in New Mexico and India. Furthermore, I acknowledge that he has studied these healing arts informally for more than 3000 hours and is a Diplomate (D.Ayur) of the American Ayurvedic Association.

He desires to teach and share with me the Buddhist Healing Arts dharmic and karmic principles and practices and he encourages me to coordinate them with my conventional health care licensed by the state. I agree to consult regularly with my usual doctor or primary care provider and do all things necessary to insure my continued good health. We declare that the Pastoral Counselor’s gentle means of Buddhist Healing Arts and my willingly receiving these services is “the practice of religious tenets of a church (Medicine Buddha Healing Center) in the ministration to the sick or suffering by mental or spiritual means as provided by the law”).
I, _________________________________, would like to receive a consultation (pastoral counseling) from the Medicine Buddha Healing Center and/or its appointed Pastoral Counselor, the monk Venerable ________________, regarding the Buddhist Healing Arts and to receive their sacerdotal spiritual healing assessment, recommendations of, giving of, and administration of diet, herbs, exercise, cleansing and rejuvenation programs for the body, mind, and consciousness (spirit).

We both agree that these services are provided as free-of-charge religious practices and tenets of the Medicine Buddha Healing Center and hence they are constitutionally protected under the First, Fourth, Sixth and Ninth Amendments of the United States Constitution. (1st Amendment: “Congress shall make no law respecting an establishment of religion, or prohibiting the free exercise thereof, or abridging the freedom of speech…”)

I, _________________________________, agree to have the consultation digitally audio recorded and to allow all of the information on the audio and on this questionnaire to be put into a public (on the Internet) educational anonymous database / correspondence course with my name, address, e-mail and phone number removed. I agree to have a picture taken of my face, hands, my tongue and my ears. The digital audio recording, and the pictures of my tongue, hands and ears will be included in the database / distance learning course. The picture of my face will remain confidential and not shared with others without my express written permission. This database / course will be used for free educational and research purposes to spread the dharmic teachings of these ancient spiritual healing arts by looking at their modern day manifestations. By filling out this questionnaire I agree to these terms and consent to freely provide the information on this form, which is required for pastoral counseling and is an integral part of ancient Buddhist religious tenets and practices described in various scriptures and their commentaries.

ARTICLE IX, U.S. CONSTITUTION
“The enumeration in the Constitution, of certain rights, shall not be construed to deny or disparage other rights retained by the People.” Under the Ninth Amendment to the Constitution of the United States, I retain the right to freedom of choice in health lifestyle education services and to pastoral counseling (as defined above). This includes the right to choose my diet, and to obtain, purchase and use any therapy, regimen, modality, remedy or product suggested by a therapist, doctor or any other practitioner, educator, or Pastoral Counselor of my choice. The enumeration in this Declaration of these rights shall not be construed to deny or disparage other rights retained by us, or our right to amend this Declaration at any time.

CONSTRUCTIVE NOTICE:
Notice is hereby given to any person(s) or governmental authority who receives a copy of this Declaration and who, acting under the color of the law, intentionally interferes with the free exercise of the rights retained by us under the First, Fourth, Sixth and Ninth Amendments, that they may be in violation of our civil and constitutional rights, Title 42, U.S.C. 1983 et seq. and Title 18, Section 241.

We declare all rights reserved without prejudice. I have read, understood and agree to the above.

The monk Venerable ________________, Medicine Buddha Healing Center, and I declare all rights reserved without prejudice. By filling out this form I acknowledge I would like to receive a no-obligation Buddhist Ayurvedic pastoral counseling informational consultation with the monk Venerable ________________.

Clinical Intake Form - Patient Medical History Intake Interview Questionnaires:
Please patiently keep track of each of the MP3 patient consultations as if they were your very own patients.
Use the short two page version of the Intake Form below -- 1st page is for Health Assessment-History and 2nd page
is for the Customized Herbal Formula Recommendations. Be sure to print off at least 108 copies of the forms.
To pass your Oral Exam for Level 1 to receive a Certificate you are required to listen to and document on paper
more than 100 hours of MP3 consultations or 108 individual MP3 recording of 108 patients -- which ever comes first. While commuting, traveling or during other opportunities, it is useful to listen to as many MP3 consultations as you can by doing "mobile learning" via your digital audio player or other mobile device (comparison of portable players on the market).
The Intake Form is key during the medical history and clinical examination since it helps to categorize the chief complaint, and all of the myriad symptoms and clinical signs into an Ayurvedic diagnosis along the lines of vata, pitta, kapha, ama-agni, seven dhatus, three mala (urine, feces, sweat), ojas-tejas-prana and other key classifications.

Please be aware as you listen to the MP3 clinical patient consultations of how Dhatu Dushti (http://www.bhaisajyaguru.com/Clinical-Ayurvedic-Therapist-Distance-Learning-Ayurveda-Correspondence-Course-index.htm#Dhatu_Dushti_Vata-Pitta-Kapha-Disease-of-Tissues-Organs) is manifesting in the patients -- what stage of tissue disturbance is clinically manifesting (click here for a full explanation for beginners: http://www.bhaisajyaguru.com/Clinical-Ayurvedic-Therapist-Distance-Learning-Ayurveda-Correspondence-Course-index.htm#Dhatu_Dushti_Vata-Pitta-Kapha-Disease-of-Tissues-Organs). This means that excess vata, and/or pitta and/or kapha dosha (may be all three doshas in serious cases) enters into the tissue systems and their respective organs causing disturbance in agni (metabolism - vata vishama, pitta tikshna, or kapha manda), creation of ama (toxins and undigested qualities in a vata, pitta and/or kapha form), and disease due to excess or inappropriate build-up of the doshas attributes and their effects (guna-karma) of vata-pitta-kapha.
