TRUSTEES OF THE STATE LUNATIC HOSPITAL, 
AT WORCESTER. 



To His Excellency the Governor, and the Honorable Council : 

The Trustees of the Worcester Hospital for the Insane, in 
presenting their Annual Report, beg leave to say that the hos- 
pital is now in excellent condition, and the result of the 
treatment of patients under the care of our Superintendent 
compares favorably with that of any previous year. 

The importance of sending patients in the early stages of 
disease will be seen by the favorable result in such cases ; and 
too much cannot be said to induce their friends to make early 
application for the treatment of those who suffer from mental 
disease. 

Improvements have been made in this hospital during the 
last four or five years that add much to the comfort and welfare 
of patients — all unnecessary restraint is dispensed with, and 
facilities for exercise and amusement within doors furnished, 
besides useful and agreeable employment and recreation out of 
doors, in good weather. 

There yet remain to be added workshops for those who wish 
employment in seasons and during weather when work in the 
open air cannot be had. This subject is now occupying the 
attention of those interested in the treatment of insanity. 
Amusement, alone, is not enough for our industrious people, 
to whom useful labor, not carried to excess, is necessary for 
pleasure and health. 

The financial condition of the hospital will be seen by the 
report of the Treasurer. The amount due for the board of

patients is more than enough to pay the indebtedness, and it is 
expected that in another year a sufficient surplus will remain 
to carry on the affairs of the institution without borrowing in 
anticipation of payments due for board. Some more efficient 
measures must be introduced to enforce the payment of just 
dues without allowing them to accumulate till they become 
troublesome to the treasurer and embarrassing to the parties 
who are delinquent. 

One of our Board has responded to his country's call for 
assistance in its hour of danger, and is now with the army on 
the Potomac, where his professional services are but too much 
needed. It is hoped that a speedy return of peace may restore 
him to his duties here.

SUPERINTENDENT'S REPORT 



To the Trustees of the State Lunatic Hospital, at Worcester : 

Gentlemen, — In compliance with the by-laws of the State 
Lunatic Hospital, and the statute laws of the Commonwealth, 
I respectfully present to you the following record of the history 
of the hospital during the year now closed. 

In looking back over the labors of the year, we hope it may 
be found that our efforts have been crowned with some degree 
of success. 

Whatever of good has been accomplished, must be mainly 
attributed to your constant oversight and wise direction in all 
the affairs of the hospital. 

By your frequent visits and thorough examinations of all the 
departments of the institution, you have been able to give 
counsel and assistance, upon which the good results of the year 
have chiefly depended. 

The general health of the patients lias been good, with 
complete exemption from all epidemic diseases. 

The hospital has accommodations for three hundred patients. 
The following tables will show that during the whole year we 
have been considerably crowded.

It will be seen by the foregoing table that one hundred and 
fifty-one patients have been discharged from the care and 
custody of the hospital as recovered. The cause of their 
commitment had ceased to exist, and they were returned to 
their homes and friends restored to mental and physical health. 

Thirty-five have been discharged from the hospital improved. 
Several of these were so much improved that they are regarded 
by their friends as well, and they have returned to their former 
duties and stations in society. 

Some of those who have been discharged are periodically 
insane, having long intervals of apparent health, during which 
they remain at their homes, and are able to perform all the 
ordinary duties of life. 

This number, however, has been so small during the year, as 
not to materially affect the per cent, of recoveries. 

The whole number admitted during the year was two hundred 
and fifty-one, of whom one hundred and twenty-seven were 
males, and one hundred and twenty-four were females. 

The whole number under treatment during the year was five 
hundred and eighty-three, of whom two hundred and eighty- 
two were males, and three hundred and one were females.

The whole number discharged during the year was two 
hundred and four, of whom ninety-eight were males, and one 
hundred and six were females. 

The whole number remaining in the hospital at the close of 
the year was three hundred and seventy-nine, of whom one 
hundred and eighty-four were males., and one hundred and 
ninety-five were females. 

You will perceive by the foregoing statement that the recov- 
eries were in the ratio of fifty-two and one-fifth per cent, to the 
whole number admitted, or twenty-two and one-half per cent, 
to the whole number under treatment, and more than one 
hundred per cent, to the number of those whose insanity had 
existed for a period less than one year. 

There has been during the year the usual amount of sickness, 
confined almost entirely to the habitually feeble, demented, and 
long insane patients. In the winter and spring there were a 
few cases of influenza, and two or three cases of pneumonia. 
During the latter part of summer there were several cases of 
diarrhoea and dysentery, generally, however, of a mild type, 
and they all yielded readily to the usual remedies. 

The mortality of the patients has been confined chiefly to 
cases of consumption, epilepsy, palsy, and old age.

The foregoing table shows that you have discharged from 
the care and custody of the hospital, in the course of the year,

eighty-eight patients, who had no settlement in this Common- 
wealth. 

Of the three hundred and seventy-nine patients remaining in 
the hospital at the close of the year, one hundred and fifty-six 
are supposed to have no settlement in the Commonwealth, one 
hundred and twenty of whom are foreigners, and thirty-six are 
of American birth. 

Very few of the patients have been removed to almshouses 
during the year, and but few have been discharged except to 
the care and custody of friends and relatives. 

Occasionally some one has been assisted by the Alien 
Commissioners to reach their friends in other States, and places 
have been found for others where they could earn . their own 
living. 

Before removing any patient to another State, the wishes of 
the friends have always been consulted before any steps have 
been taken to procure a discharge from the hospital.

Forty-seven males and forty-three females, when admitted, 
were suffering from recently developed mania, in its ordinary 
form. 

Sixteen males and thirteen females had for a long period 
suffered all the symptoms of mania, and are in the table set 
down as cases of chronic mania. 

Twenty-seven patients have been admitted who were suffering 
from epilepsy.

Little can be said in an annual report respecting the causes 
of insanity. They may be remote and only slightly predisposing 
the individual to the influence of the disease, or they may be 
immediate and exciting to a greater degree. 

Among the remote causes are constitutional predisposition, 
hereditary or otherwise. The influence of education, moral

and physical, the peculiarities of organization and previous 
attacks of disease seriously affecting the action of the brain. 

Among the immediate and exciting causes, are domestic 
affliction, reverses of fortune, disappointments, jealousy, and 
unrestrained anger. Th« exciting causes may also be physical, 
as irregular menstruation, consequence of parturition, apoplexy, 
blows on the head, and abuse of strong drink. 

In making up the foregoing table, all the causes mentioned 
are found in the records of the hospital, comprising a list of the 
names of more than six thousand patients, and embracing a 
period of more than twenty-eight years.

Four patients died during the year of old age — two males and 
two females — all of whom were upwards of eighty years of age. 

Two were between the ages of seventy and eighty years, 
and two between sixty and seventy years of age. 

One of those who died during the year had been an inmate 
of the hospital eighteen years ; two had resided in the hospital 
more than eight years, and three more than seven years each. 

Four of those who died during the year were brought to the 
hospital in such a feeble state of health that they could not sit 
up at all, and did not recruit in the least, although they 
lingered along several weeks.

From the non-completion of the plan, the additional work- 
shops, mentioned in my last report, have not been provided for 
the male patients. This, however, has not been as yet a practi- 
cal disadvantage. A very large proportion of our inmates are 
and must be from agricultural districts ; and where they are 
not farmers or farm laborers, they are, when calm and sufficiently 
vigorous, all capable of undertaking garden and ordinary field 
work, and derive benefit from exchanging the sedentary posture 
and confined atmosphere of a shop or manufactory for muscular 
exertion in the open air. The presence of mechanics in such 
an institution is of course highly useful and profitable, and in 
many cases it is judicious to employ patients in their own craft ; 
but there are unquestionably greater curative powers in the 
labor of the farm than in the shop. It is a mistake, also, to 
suppose that the kind of work which answers well in one insti- 
tution should answer equally well in another. Practically, it will 
be found, that in one hospital weaving is most successfully and 
beneficially carried on, in another the making of mats, in a third, 
garden or field labor. 

In institutions near large towns or cities, the handicrafts 
will probably be generally found to succeed best ; while those 
in the centre of agricultural districts, field labor will be most 
beneficial. 

The character of the work should be regulated partly by the 
prevalent occupations of the neighborhood in which the hospital 
is situated ; partly by the previous occupation of the patients ; 
partly by a consideration of the kind of work likely to be 
physically most suitable to individual patients. 

A variety of labor is absolutely necessary for the well being 
of any large number of patients. At the same time there is no 
employment so generally acceptable, none so serviceable in 
restoring physical health, as that of the farm or garden. 

In conducting the affairs of the hospital during the year, 
every effort has been made to provide labor for such patients as 
could be induced to engage in it.

It will be seen by the above statement, that the means 
employed for warming and ventilating the various parts of the

institution continue to give satisfactory results in an economical 
point of view. 

The various wards are always comfortably warmed during 
the cold weather, by a free circulation of fresh, warm air, and 
made cool and comfortable in the heat of summer by forcing 
into all parts of the house a constant supply of pure air from 
without. 

It is proper to mention in this connection the various sources 
of amusement and recreation afforded to the inmates as a 
means of relief and restoration. 

All the amusements and pastimes of former years have been 
kept up, and every healthy out-of-door exercise or harmless 
in-door game that could be thought of, has been put in 
requisition. 

The exercise of the mental faculties of the patients has been 
stimulated to some degree, and much recreation has been 
afforded by a course of lectures, by a series of concerts, by 
tableaux vivante, and by magic lantern scenes. Daily walks 
and drives, social parties, and during the winter skating and 
coasting parties have, as usual, been carried on with great 
animation. 

The chapel services are still performed by Rev. Samuel 
Souther, in a highly acceptable and satisfactory manner, and 
we have every reason to believe that the patients are comforted 
and improved by his ministrations. 

The patients' library continues to increase, partly by donations 
from friends, partly by purchase. In behalf of the inmates I 
desire to express my sense of obligation to the many kind friends 
who have assisted us during the year in our lectures, concerts, 
and other entertainments designed to amuse and instruct those 
placed under our care. 

I also desire to tender my grateful acknowledgments to those 
who have so generously made donations of books, maps and 
pictures, thus contributing largely to the welfare of the 
inmates. 

I am under renewed obligations to the publishers of news-

It is my duty to record my personal obligations to Doctors 
Rice and Prentiss for their devotion to the interests of the 
institution, and the cheerfulness with which they have performed 
all their duties. 

In glancing back over the records of the year, we feel that 
all our efforts for the welfare and improvement of those placed 
under us have been most signally blessed. 

The year has been one of health and prosperity. We have 
been visited by no calamity, and we are confident that much 
comfort and happiness has been secured to the unfortunate

TERMS AND FORMS OP ADMISSION, &o. 



ADMISSION OF PATIENTS. 

Inquiries are often made by letter and otherwise in reference to the 
course to be pursued in order to obtain admission for the insane to the 
hospital, and what are the expenses of support, &c. 

To entitle insane persons to the care of the hospital it is necessary 
that they be committed by order of the court of probate, or by any other 
court having jurisdiction in the town or county in which the insane 
person resides, or by two justices of the peace, one of which shall be a 
justice of the quorum. 

Patients may also be committed by their friends if satisfactory bonds 
are given for their support while in the hospital. 

Overseers of the poor may also commit the insane poor of the town 
in which they have jurisdiction, by giving bonds in behalf of the town 
for the support of the patient while in the hospital. 

When it is desirable that an insane person should be committed by 
the court of probate, any one interested may make petition to the judge 
of probate for the removal of the lunatic to the hospital. 

An acknowledgment of notice of the petition from one of the select- 
men of the town in which the lunatic resides must also be presented 
with the petition. 



FORM OF OVERSEERS BOND. 

State Lunatic Hospital at Worcester. 

Whereas of , in the county 

of , has been admitted a boarder in the State Lunatic 

Hospital at Worcester, we 

, a majority of the Overseers of the Poor of the 
town of , in the county of , in behalf 

of the inhabitants of said town, do hereby promise 

Treasurer of said hospital, to pay him or his sue-

cessor in said office, the sum dollar and 

cents per week for the board of said 

so long as he shall continue a boarder in said 
hospital, with such extra charges as may be occasioned by 
requiring more than ordinary care and attention, to provide for 
suitable clothing, and to pay for all such necessary articles 
of clothing as shall be procured for by the Steward of the 

hospital, and to remove from said hospital whenever the room 

occupied by shall be required for a class of patients having 

preference by law, or in the opinion of the Superintendent, to be received 
into said hospital ; and if he should be removed at the request of 

before the expiration of six calendar months after 
reception, to pay board for twenty-six weeks, unless he should be 
sooner cured. Also to pay, not exceeding fifty dollars, for all damages 

he may do to the furniture, and other property of said hospital, and 
for reasonable charges in case of elopement, and funeral charges in case 
of death. Payment to be made quarterly and at the time of removal? 
with interest on each bill from and after the time it becomes due.

Overseers of the Poor 

of the 
Town of 



In conformity to the laws of the Commonwealth, the Trustees are 
required at their annual meeting to establish the price of board. The 
expense for the ensuing year will be at the rate of three dollars per 
week for the first six months after the commitment of a patient, and two 
dollars and seventy-five cents per week after the expiration of six 
months. All necessary clothing must be supplied by the friends of the 
patients. 

Clothing will be supplied at the hospital if desirable and charged in 
the bills at cost. 

Damages done to the furniture and other property to the amount of 
fifty dollars may also be charged. 

Reasonable charges will be made in case of elopement, and funeral 
charges in case of death. 

All bills are collected by the Treasurer quarterly, or interest charged 
on the same after becoming due. 

Bills become due on the first of January, April, July, and October, 
and when the patient leaves the hospital.
