INTRODUCTION
The thirty-eighth annual report, Part II, of the PubHc Health
Statistics Section, formerly known as the Bureau of Vital Statis-
tics, presents tabular information regarding North Carolina's
population, births, deaths, and stillbirths. The majority of the
tabulations are by place of residence; selected tabulations, in
whole or in part, are given by place of occurrence. Communica-
ble disease morbidity data are shown in Part I of the annual
report of the section which is printed separately.
Reports have been published annually since 1914 with the
exception of the years 1918 and 1919.
Certificates of births, deaths, and stillbirths occurring in 1953
that were filed in 1953 and the first three months of 1954 are
included in this report. It is estimated that vital events occur-
ring in 1953 but reported after the first three months of 1954
will comprise less than two per cent of the total 1953 events.
There are strong indications that registration in the state has
been improving in recent years. The certificates are more com-
plete and accurate and on the average have been filed more
promptly.
Again in 1953 a tabulation was made of all birth and death cer-
tificates filed in 1953 by year of occurrence (see 1950 or 1951
annual report for explanation of interpretation). This was
done in an effort to determine the per cent of certificates filed
in time to be included in an annual report as compared with
those filed too late to be included. The results of this study are
shown in Table 8 of this report. This tabulation was prepared
by county of residence in order to make available a correction
factor that could be used by each county to more accurately
calculate the natality and mortality rates.
Nat edit II
In 1953 there were 111,622 live births occurring to mothers
who were residents of the state. All births were allocated to
the place of residence of the mother regardless of the place of
birth or the length of residence. Births occurring in hospitals,
sanatoria, and all other institutions, were allocated to the place
where the mother lived. Births occurring on military bases to

military personnel were allocated according to the residence
shown on the certificate. The crude birth rate was 26.4 births
per 1,000 population. There were 74,038 white births and 37,584
nonwhite births. For the seventh consecutive year, over 100,000
births were recorded.
This state defines a stillbirth as a fetus showing no evidence
of life after complete birth if the twentieth week of gestation
has been reached. There were 2,231 resident stillbirths reported
resulting in a rate of 20.0 stillbirths per 1,000 live births. If still-
births were considered as a single cause of death, they would rank
fourth between cancer and influenza and pneumonia as a leading
cause of death.
Premature Births
In order to achieve uniformity in the classification of prema-
ture infants in need of special care, and comparability of statis-
tical data in hospitals and in studies of such infants, the definition
of the American Academy of Pediatrics has been adopted ; that
is, a premature infant weighing 2500 grams or less at birth
regardless of the period of gestation.
In 1953 there were 9,463 resident premature live births re-
ported among 111,622 live births. Birth weight was reported
on all but 323 certificates filed in 1953. Consequently, at least
8.5 per cent of the newborn babies weighed less than 5 pounds
and 9 ounces at birth. The number of premature infants by
birth weight and age of mother are shown below :

Mortality
There were 31,640 resident deaths recorded in 1953. The crude
death rate was 7.5 deaths per 1,000 estimated population. Deaths
were allocated to the place where the deceased resided unless
the place of death item indicated that the deceased had been in
the place of death for one year or more. In such cases they were
allocated to the place of death, unless the death occurred in an
institution. If this was the case, they were allocated to the place
of residence, regardless of the length of stay in the institution.
Diseases of the heart, as they have been for the past 32 years,
were responsible for more deaths than any other cause, account-
ing for approximately one-third of all deaths for the year.
All mortality statistics shown by cause in this report are classi-
fied according to the Sixth Decennial Revision of the Interna-
tional Lists of Diseases and Causes of Death. This is the fifth
year that this revised statistical code has been applied to the
causes of death; therefore, some attention must be given to the
effects of the revision on certain causes of death before com-
parison with previous years can be made accurately.
The most important feature of this revised classification pro-
cedure is the emphasis that is placed on the underlying cause of
death, as determined by the attending physician. The physician's
statement is used as the determining factor when two or more
causes of death are entered. Approximately 75 per cent of the

death certificates have two or more causes entered in the med-
ical certification section. For the decade, 1939-1948, the selec-
tion of the cause of death for tabulation was made according to
joint-cause rules which established priority of cause based on
certain factors rather than on the physician's opinion.
In Table 14, regarding deaths from selected causes, 1916-1953,
it is necessary to take into consideration the effect of the revision
of the International List and of the joint cause of death coding
procedure. A comparability ratio may be obtained upon request.
This ratio is calculated from the approximate change that has
taken place due to coding revisions between the periods, 1949-
1953 and prior years, 1939-1948. This ratio was obtained by
classifying deaths according to the Fifth Revision of the Inter-
national Code as well as by the Sixth Revision and then basing
the ratio on the changes derived.
Hospital Data
All tables in this report which reflect events occurring in hos-
pitals are based on the general and allied hospital definition of
the North Carolina Medical Care Commission. A hospital is
defined as a place where two or more non-related individuals
reside 24 hours or more on the premises and receive medical care
over and above any domiciliary care.
Infant and Maternal Mortality
There were 3,642 infant deaths and 112 maternal deaths re-
ported. The resultant infant and maternal deaths per 1,000 live
births were 32.6 and 1.0 respectively. The maternal mortality
rate was the lowest on record. The actual number of maternal
deaths represent a new low dating back to 1914.
In 1918, there was an all-time high rate of 101.8 infant deaths
per 1,000 live births. The infant mortality rate was almost three
times as high thirty-five years ago as it was in 1953 while the
maternal rate was ten times as high at that time.
Rates
All rates shown in this report are crude rates unless otherwise
specified. Such rates have not been adjusted for differences in
the age, sex, color, rural or urban distribution of the population,
or for under-registration or late reporting. Such factors should
be kept in mind when comparing two different areas or time
periods.
